Summary

Names and zip code counts go. The yearly total of forms sent stays.

The bill bars the Department of State Health Services from keeping the names of people who request a vaccine exemption affidavit form, and from keeping counts of those forms broken down by zip code. It touches parents and others who request exemption forms, and the state health department that sends them out. The department can still track and report the total number of forms sent each year, but it can no longer keep a list of who asked or map requests by zip code, starting September 1, 2025.
