relating to an exemption from required immunizations for school enrollment.

Caption from analyzed text · version E.

Summary

Printable online. Mail still works.

The bill makes the state health department post the school vaccine exemption affidavit online in a printable format, while still letting people request it by mail. It affects parents and guardians who claim a vaccine exemption for their children, along with the Department of State Health Services. The form no longer needs an anti-copy seal, and nobody has to give personal information to get one. The department no longer has to count or report to the Legislature how many forms it sends out each year, and it may not keep a record of who downloads, requests, or submits a form.
