Summary

Spouses, not husband and wife. Any applicant can change a surname.

The bill rewrites two state marriage forms so they use gender-neutral wording. On the marriage license application and the informal marriage declaration, the 'woman's maiden surname' line becomes the surname of any applicant who plans to change their name, and the informal marriage oath says 'spouses' instead of 'husband and wife.' County clerks and couples filling out these forms are affected starting September 1, 2025, and the rest of what the forms ask stays the same.
