Summary

The department counts website visits but can’t record names.

The bill makes the state health department post the vaccine exemption affidavit form on its website and in all public schools, and drops the rule that the form carry a seal or anti-copy security device. It touches parents, guardians and adults who claim an exemption from required immunizations, and the schools that accept those forms. Instead of requesting a form from the state, people can download it, and the department counts website accesses while being barred from recording the names of people who download or access the form.
